DescriptionJob SummaryUnder the direct supervision of a licensed pharmacist, the Medication History Registered Pharmacy Technician participates in direct patient interviews and obtaining medication histories for patients being admitted to the hospital. Obtains medication history information directly from the patient or their caregiver(s) and gathers supporting information from the patient's physician(s), outpatient pharmacy records, or previous inpatient admissions, if necessary.
