Primary Duties and Responsibilities:

Medication Reconciliation Pharmacy Technician Duties:
? Conduct in-person or phone meetings with patients to update medication lists prior to their provider appointments, ensuring accuracy and completeness of the medication list.
? Practices as a pharmacy technician in accordance with all federal, state, DFCI and Joint Commission policies, regulations and standards.
? Create and update lists of current medications, document changes in dose, frequency, or route, and flag medications for removal to prevent harmful drug interactions.
? Communicate drug-drug interactions or other medication changes with pharmacists and providers to maintain patient safety. 
? Manage medication lists for medications listed from external entities, ensuring comprehensive and accurate records.
? Utilizes multiple systems such as EHR (Electronic Health Record, Inventory Management Systems and other systems to complete and document duties.  Troubleshoot as necessary. 
? Trains other employees as assigned.
? Communicates and escalates issues or problems.
? May be asked to provide cross coverage in the hospital outpatient/specialty pharmacy as needed to maintain seamless patient care.
? Performs other duties as assigned.

? Ability to effectively work within a collaborative team-focused environment.
? Ability to effectively communicate both orally and in writing.
? Ability to treat all others (fellow pharmacy staff, patients, nurses, providers, support personnel, etc.) with the utmost professionalism and respect. 
? Ability to independently troubleshoot issues and assist in resolution or escalation to the appropriate resource.
? Participate in department meetings and functions to promote a successful and efficient pharmacy workgroup.
? Demonstrated critical thinking skills (ability to analyze information objectively and make a reasoned judgment).
? Strong attention to detail.  

Minimum Job Qualifications:

Minimum Education: 
? High School graduate or equivalent.

Minimum Experience:
? Two years of prior work as a pharmacy technician in a retail or hospital environment required.
? Experience with oncology medications preferred.

License/Certification/Registration Required:

? Massachusetts Board of Pharmacy Registration as a Pharmacy Technician required.  National Pharmacy Technician Certification (PTCB) must be obtained within 1 years of hire date.
